The following forms are used to apply for licensure or to make changes relating to an existing licence.

Confidentiality Agreement Form (PDF)

Agreement to be completed by all those who apply to take the Jurisprudence Exam.

Complete and submit this form to apply for any type of licence with the College.

Licence Resignation and Application for Emeritus Status (PDF)

Licensed members wishing to resign from practice complete and submit the Resignation Form. Eligible members may also be granted Emeritus Status.

Complete and submit a Letter of Standing Request to have a letter of standing sent to another governing body.

Complete and submit the Application for Name Change if you wish to change your name from the name that appears on the Public Register.
