The following forms are used for activities related to new and existing veterinary facilities.
The name of a veterinary facility requires the approval of the Registrar or the Accreditation Committee. Complete and submit the Application for a Facility Name to begin the accreditation process. Apply Online
Veterinary facilities are inspected at opening, when there is a change in ownership, when a facility relocates and when there are changes to the facility. Complete and submit the Application for Inspection and Accreditation of a Veterinary Facility in these situations. Apply Online
The College must be notified when there is a change in directorship of a facility. Complete and submit the Notification of Change in Directorship of an Accredited Facility. Download Form
The College must be notified if a veterinary facility is closing. Complete and submit the Notification of Facility Closure. Apply Online
