Licence reinstated; limited to food-producing animal practice, excluding equines; prohibited from purchasing, prescribing, possessing, dispensing or administering any controlled substance, narcotic or targeted drugs; must complete 15 hrs of CE related to food-producing and large animal practice within the first year of issuance of licence; must meet with a mentor for 3 hours, once a month, for 1 year; records must be reviewed up to 8 times over the first two years of licensure; must enroll in a substance-use monitoring program
